To place the initial order, email a completed requisition form for the patient to homecollection@cirrusdx.com. Include the patient’s address and telephone number.
The collection kit with return shipping and instructions will ship the next business day (Monday-Friday) as long as the request is received by 3pm ET.
The patient’s name and date of birth will be on the sample collection device in the home kit. Please remind the patient to provide the collection date when shipping their sample.
Patient’s sample is returned via FedEx using the provided return packaging.
Samples will be delivered overnight (Monday – Saturday) to the lab for testing. It’s that easy!
